If you received your COVID-19 vaccinations in Arizona, digital records are available. Although this step is optional, obtaining a digital vaccination record may help you if your host country requires you to show proof of vaccination. This way, you won't have to worry about keeping your vaccination card on you at all times. Complete the steps below to access and download your digital vaccine record.

  1. Log into AZMyIR. Create an account if you do not already have one.

  2. Once logged into the site, select the COVID-19 Get your proof of immunization tab on the left-hand side.

  3. A SMART Health Card with a QR code and your COVID-19 vaccination information is displayed.

  4. This QR code and associated information can be printed or stored on your cell phone. Follow the manufacturer’s instructions for storing this record digitally to your Android or Apple device.
