The deadline to submit your community engagement projects has been extended through Monday, Oct. 31. It should take no more than five to 10 minutes of your time to complete this webform.

This data collection is part of a self-study to help us understand the breadth and depth of our community engagement, develop more effective and equitable engagement practices, and recognize individuals and programs across the University that make a difference in our communities.

We are seeking information about your community-engaged teaching, research, outreach and service at the University. Examples may include:

  • Collaborating with a community partner on a course you are teaching.
  • Offering a community-based internship.
  • Helping your department or unit with a community service project.
  • Participating in any number of other forms of community engagement. 

This information is being conducted by a diverse working group leading an initiative to advance the University's community engagement efforts and receive the Carnegie Foundation's Elective Classification for Community Engagement.

Before completing this form, please note:

  • We will not contact your community partners without your permission. These data are used to identify and describe the breadth of community-engaged work across campus.
  • The form can be completed by someone else on your behalf.
  • You'll need to log in using your NetID to verify that you are a University employee or student to complete the form.
  • Your responses will be saved as they are entered. Use the same form link to return at a later time, if needed.
  • Be prepared to answer the following questions:
    • Name of the partnership/collaboration
    • Name, contact information and location of primary community partner
    • Name of University department/unit involved
    • Purpose of the project (500 words or less)
    • One example as to how reciprocity is enacted
    • Length of the partnership
    • Number of faculty, staff and/or students involved (estimate)
    • Funding source (if applicable)
    • Impact on the community and the University (500 words or less)
