Subject: Get a free seasonal flu shot – help protect your community
Date: Sep 11, 2020

Help protect the most vulnerable members of our community by getting a flu shot this fall. Immunizations for the seasonal influenza virus are especially important during the coronavirus pandemic, to help reduce the load on local emergency departments and intensive care units.

If you're currently working on campus, please take advantage of free flu shot clinics offered by Life & Work Connections, in collaboration with the Arizona Department of Administration Benefits and the Healthwaves corporate wellness team. These clinics are open to all benefits-eligible employees and their medical dependents, as well as University of Arizona retirees.

Working remotely? No need to drive or ride in for your free shot! You can visit an ADOA or Healthwaves clinic closer to home – with sites available around the state – or receive your free flu immunization at your doctor's office or a local pharmacy. (Make sure the pharmacist bills your health insurance and not your MedImpact prescription coverage.)

Note that all dependents age 4 or older covered by your medical plan are also welcome at campus flu shot clinics.

What you need for a campus clinic
New this year: You will need to register for an appointment to receive your flu shot on campus. If you are bringing a dependent, they will need to be registered as well.

You will also need a Flu Shot Consent Form, completed with your alternate employee ID, for each person receiving an immunization. (If you don't have a printer at home, the clinics will have extra forms on hand – but expect a delay if you have to fill yours out on-site.)

Dependents should bring their health insurance cards to the appointment, and the cards must have their names on them.

All clinics will be held outdoors this year to reduce crowds and increase ventilation. Facilities Management will provide hand sanitizer stations at each site.

Remember to wear your face covering at the clinic, in accordance with the University's Administrative Directive. If you're feeling sick the day of your appointment, please stay home and rebook for another day.

What you need for an ADOA or Healthwaves clinic
You will need a Flu Shot Consent Form, completed with your alternate employee ID for clinics run by ADOA or Healthwaves. If you do not have access to a printer, consider calling ahead to the clinic a few days in advance to be sure they will have extras on-site.

Some ADOA clinics require registration. Public clinics held by Healthwaves do not.
