Upload your vaccine document and help us track our progress toward campus immunity

If you are fully vaccinated, please upload your vaccine document by using the secure HIPAA-compliant Campus Health system. 

Employees who upload (or have already uploaded) their vaccine document will be automatically entered for a chance to receive exciting incentives, including:

  • Parking permit for the garage/lot of your choice
  • Private viewing of an Arizona Athletics team practice, including a meet-and-greet with the head coach 
  • Tickets to Arizona Arts Live
  • Campus Rec semester memberships

Students will also be eligible for incentives. Recipients will be selected at random and notified via email to claim their incentive. Visit the COVID-19 vaccine page to learn more or email covidresponse@arizona.edu with questions.

Not vaccinated yet?

Vaccination protects the vast majority of recipients from severe infection and transmission of the coronavirus, including the variants identified to date.

If you are in Tucson, you can receive your vaccine at locations across Pima County. Campus Health is offering the Pfizer vaccine to all University students and benefits-eligible employees. If you're outside of Tucson, check VaccineFinder for availability in your area.
